I acknowledge by submitting this agreement that upon accepting service I will be subject to the terms and conditions of utility service as contained in the Tariff Rate Schedule. I further acknowledge and agree that the terms and conditions of utility service contained in the Tariff Rate Schedule shall constitute a contract between The Utility and me and may be enforced as such. No pipe or fitting, solder or flux, which contains more than 0.2% lead can be used for the installation or repair of plumbing at any connection which provides water for human use. We or our agents may call by telephone and/or contact you using any contact information you provided regarding your account. You agree that we may place telephone calls using an automatic telephone dialing system/announcing device/an artificial or prerecorded voice. You agree that we may make such calls to a mobile telephone or other similar device. You agree that we may, for training purposes or to evaluate the quality of our service, listen to and record phone conversations you have with us.

I agree that the water service may be connected in my absence at the service address I've entered in this application. In return for The Utility connecting that service, I release, indemnify and forever hold harmless The Utility and its personnel and affiliated companies from any liability for water damage that may occur inside that residence or to those service lines due to the water service being turned on in my absence. Further, I agree that The Utility and affiliated companies may disconnect/cut off the water service should the meter or other evidence indicate that there is water being lost for any reason. In this case, it will be necessary for me to set up another appointment, during regular business hours, and at which I (or an authorized agent over age 18) am present, to have the service connected/turned on at the residence.
